Click New within Interactive Content (Content Creation) > Select Questionnaire

Questionnaire Settings

Give this questionnaire a Title.

For each question, choose an Open-Ended Question or a Simple Multi Choice Question. Configure each question with the necessary information.

Click Add Element to create a new question.

Messages

Configure messages that appear to users.

Additional Settings

Configure additional settings such as Behavioral Settings to set how users can interact with this activity.

Click Save.
